Health Information Needed to Register for School Maintaining the health and safety of the learning environment is central to the mission of the Somerville Public Schools. We comply with Massachusetts Department of Public Health regulations that require us to have the following, updated health information from students new to the Somerville Public School upon the time of enrollment. Please bring the following information with you when you register for school. Documentation should include the date and signature/stamp of the provider/clinic in an immunization book or office form. If you do not have a copy of the information at home, please present this form to your health care provider to obtain the necessary information. If a physical examination or TB screening was performed more than 1 year ago, or any information is unavailable, please bring a note from your health care provider documenting the date of a scheduled appointment to obtain the exam/screening/immunization/blood test. Information must be received within a week after the appointment in order to complete the registration process. · Physical examination performed within past (1) year for students entering grades K-12. Preschool is excluded from this requirement. · Tuberculosis risk assessment or TB skin test reading within past (1) year · Lead blood test - result and date of previous lead blood test for students enrolling in preschool and kindergarten only · Current immunization records that show the following:
