Document Requirements for Enrollment
Important Enrollment Documents
Here are the documents you will need to complete enrollment:
- Your valid state photo I.D., driver’s license, passport, or other government-issued identification
- Your child’s birth certificate
- One (1) proof of residency:
- Copy of current signed lease, incl. SHA/HUD leases
- Copy of most recent mortgage statement or deed
- Notarized residency affidavit completed by landlord or lessee, plus:
- copy of utility bill in landlord/lessee name, rent receipt, or cancelled check from rent payment
- Section 8 agreement
- One (1) proof of occupancy:
- Utility bill or work order dated within the past 60 days, including:
- gas, electric, oil, cable, cell or home phone bill
- Property tax bill
- Excise tax bill dated within the past year
- Letter from Mass Health or private health insurance agency (not bill)
- Printed pay stub
- Letter from Dept of Revenue or Dept of Transitional Assistance within 60 days
- Utility bill or work order dated within the past 60 days, including:
- Your child’s most recent immunization and health records
- A recent completed physical examination with immunization records:
- an updated one within one year of school entry will be required
- Doctor’s orders if your child will need to take medication during the

- A recent completed physical examination with immunization records:
- Prior school records (Gr. 1-12 only)
- Your child’s most recent official school transcript, and,
- Discipline report from all high schools the student attended (SHS register or transfer only)
- Other important documents as needed
- Active IEP, 504 plan, custody agreements, guardianship, court order, etc.
- Current Access Scores (for English Language Learners)
